Introduction

Scleroderma is an autoimmune disorder. Scleroderma, also called systemic sclerosis, is a group of rare diseases in which  the hardening and tightening of the skin is involved.In Scleroderma  problems in the blood vessels, internal organs and digestive tract are commonly found. Scleroderma is often categorised as limited or diffused, which refers only to the degree of skin involvement as there is too much collagen involved which further makes the skin tight and hard and this causes scleroderma.Both types can involve any of the other vascular or organ symptoms that are part of the disease.It involves many systems in the body. Localised scleroderma, also known as morphea, affects only the skin.There is no cure for scleroderma treatment can decrease the symptoms,may show progression and improve the quality of life.

There are Two Types of Scleroderma

  1. Localised Scleroderma-It only affects the skin and also affects the structures that are under the skin.
  2. Systemic Scleroderma-Is also called systemic sclerosis that affects many systems in the body.It can also damage blood vessels and internal organs like heart,lungs and kidneys,etc.
  3. This Is further divided into two additional categories-Limited and Diffuse which represents how much skin is involved in the body.

Causes

The Exact cause of Scleroderma is Not found but there are many factors which are involved in causing scleroderma as

  • Genetic factor-Scleroderma can not be inherited from one generation to another like genetic diseases but the genes can increase the chance for certain individuals to develop scleroderma and also to find out what type of scleroderma they have.
  • Environment Factors-Exposure to chemical and toxic fumes can trigger or increase the chance of scleroderma.
  • Immune factors-if the immune system of the person is abnormal or there is any inflammation in the body,it may lead to the production of too much collagen which causes scleroderma.
  • Hormonal factors-Hormonal or immunological differences between male and female can play a role in the disease as females develop more types of scleroderma than male ones. 

Symptoms

  1. Hardening,thickening or tightening of the skin-the skin becomes hard,thickened and tightened and the person may develop patches all over the body.
  2. Hair loss and less sweating-the area that is hardened and tightened becomes shiny,discoloured and has hair loss.The person loses the ability to sweat.
  3. Skin becomes dry and itchy.
  4. Skin becomes lighter or darker than the natural skin.Some may also have violet coloured skin.Darker or whiter areas are hard to touch.
  5. Salt and pepper look on skin tends to develop on areas like the upper back,chest or scalp.
  6. Stiffness in joints and difficulty in moving them.
  7. Loss of tissue beneath the skin-like Parry-Romberg Syndrome (PRS)

Diagnosis

Scleroderma is difficult to diagnose as it can be of different forms.In order to find out any autoantibodies present in blood,Antibody nuclear test(ANA)can be performed.Depending on the clinical situation,other tests are done as blood test alone can not diagnose scleroderma.

  1. Pulmonary function tests in order to find out how the lungs are working.
  2. CT Scan to evaluate the extent of lung involvement.
  3. Electrocardiogram in order to see if there are any changes in the heart muscle tissue due to scleroderma.EKG or EEG measure the electrical conductivity of the heart and also show if there is any abnormal rhythm or any damage.
  4. X ray to see any change in the body or soft tissues caused due to scleroderma.
